• The links below will download the current registration forms, as pdfs.
• The forms will be in your 'download' folder, there should be a link to this folder at the top right of the screen.
• Use the Acrobat print button (or use Ctrl+P) to print the form, the forms are two pages each, if possible please use double sided printing.
• Please complete one for the parents/carers and one for each of of your children.
• Please return it to Joan with any booking forms.
